If you would like a brochure mailed to your home, please e-mail us and ask to be added to the mailing list.

NEW: Our Extra Time Care (ETC) will be available through July 30. Please review the new SummerTime brochure for details. 

Application
Please read and carefully fill out all questions on the registration and student information forms. Also, make sure the student information form is signed and dated. Applications without a proper signature cannot be processed. Forms are available by clicking here. (Adobe Reader required) 

Payments
All registration fees (including supply fees) are due in full with your application. Payment is accepted by credit card or by check or money order made payable to Maumee Valley Country Day School. Cash will be accepted if you are applying in person. A confirmation notice is sent upon receipt of a completed application and payment. Your confirmation will contain information regarding check-in procedures.

Refunds
You may cancel up to two weeks prior to a session and will receive a full refund minus a $10 cancellation fee. All camps and classes are subject to cancellation due to insufficient enrollment. If this should occur, you will be notified and given a full refund.
